Corner Canyon Health Centers is a Residential Treatment program for adults experiencing mental health concerns of all forms. We specialize in trauma and with the LGBTQIA+ population.
The Grave Mentor is responsible for the operations during the assigned graveyard shift, ensuring client safety and maintaining a structured and supportive environment. This role will receive guidance from the Grave Lead, ensuring that shift duties are carried out effectively and efficiently. The Grave Mentor will respond to crises, and enforce adherence to policies and procedures. Additionally, the Grave Mentor is responsible for problem-solving shift dilemmas in conjunction with the Grave Lead, and maintaining accurate documentation to ensure smooth communication between shifts. The Grave Mentor plays a key role in promoting a positive team culture and ensuring adherence to trauma-informed care principles.

Training and Collaborating with other Grave Mentors on shift to be more efficient in their approach, and use of tools
Gaining a knowledge of program details, protocols and systems
Supervising the facility and maintaining client safety
First responder to crisis situations
Generating and maintaining emotional safety - protects culture for both peers and clients
Building relationships and looking for ways to create teamwork
Completing accurate, timely documentation of staff logs, incident reports, and PRN documentation
Relaying information to Grave Lead about previous shifts to oncoming shift
Following up on assigned tasks and returns and reports in a timely manner
Working with Leads, Supervisors, and House Manager for administrative and special assignments
Dispensing of medications, per CCHC protocols
Administer Urine Analysis Screenings and Accessioning
Transporting clients per CCHC protocols
Problem-solving unforeseen shift dilemmas
Updating Daily Client Schedule to include; Groups, Sessions, Urine Screenings, etc..
Attends weekly/bi-weekly staff meetings and trainings
Actively participates in the program improvement process by providing feedback to management
Remain alert and attentive the entire shift.
Experience in the behavioral health field (preferred), but not required
Crisis prevention and de-escalation experience (preferred)
Exceptional relationship skills while maintaining ethical and professional boundaries
Dependable and reliable with an ability to maintain and manage assigned schedule
Proficiency in shift documentation, compliance tracking, and reporting.
Ability to problem-solve and adapt to the needs of the facility and clients.
Must be able to pass a background screening through DHHS
Must have 5 years driving experience
